General Purpose of Job: Centralizes Agency referrals to assure that appropriate services are based on defined needs to patient/referral source. Assures that reimbursement guidelines are met. Provides document review after completion of entry of patient into agency. Essential Duties:
- This job description is intended to cover the minimum essential duties assigned on a regular basis. Associates may be asked to perform additional duties as assigned by their leader. Leadership has the right to alter or modify the duties of the position.
- Receives and assesses intake information from families, physicians, hospitals, and other community resources and refers patient(s) to the appropriate service once admission criteria is met.
- Notifies Clinical Manager of patient's acceptance for home care; and provides intake data, essential background information, and verbal orders/signed medical orders.
- Promotes/educates referral sources regarding the Sparrow Home Care services that are available, given that reimbursement criteria are met.
- Serves as a resource to hospital personnel, patients, families, physicians, home care clinicians, maintains and enhances professional expertise through appropriate educational/organization activities.
- Assists in the development of tools and collects data for monitoring agency compliance with Conditions of Participation, JCAHO standards, OASIS, and monitoring utilization of services for improving patient outcomes and agency profitability.
- Reviews plan of care and re-certification for medical necessity, clinical appropriateness and completeness. Interprets data gathered to make recommendations for change as needed to support compliance issues.
- Assists in ensuring home health coding standards are met.
- Communicates with physicians, nurses, and other disciplines to establish a coordinate home care plan.
- Supports the performance improvement process through data collections as requested, by conducting chart audits and assisting in a variety of reports, using critical thinking skills.
Job Requirements

* Nursing license in State of Michigan * Valid Drivers License and automobile insurance * Active American Heart Association BLS Certification upon hire |
Work Experience |
* Minimum of two years previous hospital/home care experience |
Education |
* Associate degree in Nursing |
Specialized Knowledge and Skills |
Required: * Ability to implement and follow policies and procedures in Agency and Health System. * Ability to assure confidentiality regarding sensitive materials. * Demonstrated excellent interpersonal/communication skills, verbal and written. * Demonstrates ability to use a keyboard as may be required to perform the essential duties of the job. Preferred: * Bachelors Degree in Nursing * Previous direct patient care in a home care agency * Utilization review |
