This position is responsible for the review of utilization data, clinical data, and referrals from care/case management for appropriateness of the hospice/palliative care program. The staff member will coordinate communication of referral with the member and PCP, coordinate accepted referrals to the hospice/palliative care unit, track/trend/document referral, and assist in the case management of the member as needed. Responsibilities also include trending referrals, acceptance onto the palliative or hospice program, and trending data regarding hospitalizations/utilization of services. All trending reports will be shared with the Supervisors of Complex Care Management.

• A minimum of two years nursing experience in hospice, oncology or similar field required
• New York State RN licensure
• Valid NYS Drivers' License, if making home visits
• National Board Certification in Hospice & Palliative Care (CHPN) preferred or obtains certification within one year of hire
