They function as a member of the departmental team which includes educating the patient, family and members of the healthcare team regarding benefits, community resources, referrals for counseling and other pertinent information. Social workers are also responsible for triaging referrals and collaborating on cases with other members of the healthcare delivery team. In addition, social workers must be able to assist the patient in a sensitive and supportive manner, while acting as an advocate on behalf of the patient.

Medical Social Services are provided by a qualified social worker in the place of patients residence and under the direction of a physician and in accordance with a plan of care. The Social Worker is a member of the Interdisciplinary group.
Primary Responsibilities:

Provides quality / coordinated home care to clients served by a Primary Care Team under the direction of the RN Supervisor and in coordination with other team members, the physician and client
Assists the physician and other IDG members in recognizing and understanding the social / mental stress and / or disorder that exacerbates the symptoms related to complex, chronic illness
Provide services that meet the State and Federal regulations; are congruent with the policies of Southwest Medical Associates and within the scope of professional practice as defined within the professional practice act
Strives to provide services to meet both internal and external customers expectations
Assumes individual responsibility for professional competence and clinical practice
Provides social services including: assist the physician and other team members in understanding the significant social and emotional factors related to the health problems that affect the patient and family / caregiver, short-term individual counseling; crisis intervention; assistance in providing information and preparation of advance directives; end of life planning issues and transfer or responsibilities regarding fiscal, legal, and health care decisions
Collaborates with the IDG members in preparing, evaluating, integrating and implementing the IDG plan of care to assure the highest quality of social services are offered to patient and family
Conducts a comprehensive assessment upon admission which includes: patient / family psychosocial status; assessment of emotional factors related to complex, chronic illness; potential risk of suicide and / or abuse or neglect; environmental resources and obstacles to maintaining safety; identification of family dynamics and communication patterns; caregivers ability to function adequately; bereavement needs; need for counseling related to risk assessment for pathological grief; special needs related to cultural diversity including communication, space, role of family members, and special traditions
Identifies family dynamics, strengths, communication patterns, coping skills, and developmental level of patient/family and obstacles to learning or ability to participate in care of patient
Identifies barriers to adhering to plan of care and assists patient and family in understanding goals of interventions, including discharge planning
Identifies support systems available to reduce stress and facilitate coping with complex, chronic care
Identifies and uses appropriate community resources and assesses patient / family ability to access them
Identifies patient/family needs when level of care changes; such as transfer to inpatient facility, assists with ensuring needs are met and assesses the ability of the patient and family to accept change in level of care. Coordinates and communicates discharge planning
Provides interventions for specific symptom relief of fear, grief, depression, and anger
Assists patient / family in assessing financial resources when appropriate
Other duties that are related to the coordination of medical management operations

Required Qualifications:

Licensed Bachelors or Master's degree-level clinician in Psychology, Social Work, Counseling or Marriage or Family Counseling, or Licensed Ph.D., or an RN with 2 or more years experience in behavioral health
NV Social Worker license free of restriction. Licenses must be active and unrestricted
Minimum three to four years experience in social work setting in home care, long term care or other Medicare / Medicaid environment
Strong knowledge of medical terminology
Proficient in using both Windows and non - Windows software
Ability in coordination of medical information and contribute to interdisciplinary team meetings
Ability to contribute to the organizations efforts to eliminate racial and ethnic disparities in organizational performance
Good organizational, oral, and written communication skills

Preferred Qualifications:

Knowledge of community resources and managed health care system
Case management experience
Experience in managed care
